me+ME: More on bullshitting

Let me continue yesterday's The gentle art of bullshitting the public for money
with some more on bullshitting, since a considerable part of what I learned the past year of reading about ME is that its treatment, conception, reception and health care have been systematically vitiated by bogosity aka cant aka crap aka bullshit:

  • The Wesselyans, Reevesians, and Bleijenbergians have whole careers based on bullshitting about ME.
  • The treatments on offer for ME - CBT and GET, for the most part, or else soul-healing crap like LP - are based on bullshit 'evidence based medicine' (bullshit laced with crap served with a sauce of cant on a bedding of bogosity).
  • The ways civil servants, health bureaucrats, politicians and lots of ordinary folks with tiny brains brainwashed by media-bullshit regard persons with ME and talk about their illness is bullshit alternated with crap, served with sanctimonious cant and very often laced liberally with plain sadism. Consider e.g. Burnley protest over medical tests for benefits claimants (and don't forget to peruse the readers' letters if you want to learn the mentality of ordinary men (*))

1. Get your own bullshit-protector

The illustration stems from dr. Speedy's Blog about ME, where it appeared on January 28, without my having seen it until today (for else I would have used it yesterday) - and the picture itself links to the source: Inventor Bill Moyers, who designed and wore it as veteran when listening to president Bush Jr. in 2004.

Now as to how to acquire the elementary principle of the depicted BS-Protector (BSP):

I link to Wikipedia - and note that many entries in the Wikipedia about philosophy, and also about sociology and psychology, are far more postmodern than agrees with me, or indeed with rationality, science and moral decency and honesty, but then no doubt I am somewhat of an intellectual aristocrat (**):

Let's now consider three examples I was mailed that involve dr. Nancy Klimas, who is, next to one of the co-authors of the CCC-criterions, that were and are important, and may and do recommend her indeed to patients with ME, very, very many things:

"Nancy Klimas, MD, Medical Director

After more than twenty years in the field as a clinician, researcher, leader, advocate and educator, Dr. Nancy Klimas decided it was time to do even more.  She decided that the only way to see more patients and train more doctors was to open a nationwide network of clinics that would take seriously both the treatment of patients with Chronic Fatigue Syndrome (CFS) but also begin to fill the void of trained clinicians by training them herself.  These clinics would follow the methodologies created by Dr. Klimas in her current clinic, using solid, evidenced- based medicine to treat this unique population.
 
A leader in the field of Immunology and an expert in Chronic Fatigue Syndrome (CFS), Dr. Klimas is the immediate past President of the International Association for Chronic Fatigue Syndrome. A Professor of Medicine, Psychology, Microbiology and Immunology at the University of Miami, Miller School of Medicine, Dr. Klimas is principal investigator of one of the three National Institutes of Health sponsored CFS Research Centers.  At the University of Miami, Dr. Klimas leads a multidisciplinary research team representing the fields of immunology, autonomic medicine, neuroendocrinology, behavioral psychology, rheumatology, nutrition, and exercise physiology."

Now recently she has co-authored a - so called - study ('evidence based medicine', indeed, it is called) of whom a Dr. Antoni - whose doctorate is in the - so called - science of psychology ('evidence based', I must presume) with this conclusion, in brief, in professors dr. Klimas own words

Dr Antoni’s study shows that (..) CBT helps people with the illness (ME/CFS)

This study put quite a few supporters of professors dr. Klimas (who I write as "professors" because she is that, in the singular, in no less than four sciences) in a grave quandary - a State Of Cognitive Dissonance (<- Wikipedia), no less - because most persons with ME know that CBT does not help people with ME, which is indeed a claim by professors White and Wessely, that also completely contradicts the WHO's classification of ME as a neurological disease, that is specifically not a psychiatric disease.

Professors dr. Klimas and co-authors see no problem in that whatsoever, that I have seen, and seem to consider it a no-brainer that - of course, you dumbo patients! - We Need To Do Evidence Based Research Into CBT.

Probably, they all believe it is both moral and scientific, and anyway they suffer neither the consequences nor pick up the bill, while getting paid well - and it also avoids hassles with such worthies as professor Peter Denton White and professor Simon Wessely.

But some patients with ME were not completely convinced by professors Klimas story, and even dared to say so, which perhaps they should be entitled to, among a species that calles itself "sapiens" and prides itself on its rationality.

Not so! And if I may, perhaps, have sounded just a tad cynical, I should wash my mouth with carbolic soap, it seems suggested by a mighty mind and gigantic moral hero, that accordingly calls itself "Anon":

Dr Klimas has a 25 years experience with treating ME/CFS. She is compassionate and truly understands the disease and will fight the patients doctors to death. She has volunteered on a number of committees like CFSAC and has a number of relationships with key people in the community to help ME/CFS patients. For that I say thank you.

Instead of tarnishing the reputation of one of our rare, awesome doctors, one would be better off trying to be an advocate for our cause.

See?!! That settles it, doesn't it? In case you still doubt: Not only did a possible co-worker, nurse or secretary of professors Klimas enter the fray, so did a "nklimas", whom I shall presume to be the professors herself, since it was on the ME Association's site, and they do not seem to doubt that attribution - and I give it as I received these items:

"Dr White challenged me in a meeting a year ago saying nothing else had been published to deny this finding. SO now you have a publication, written by a psychologist and well regarded CBT expert to use when you want to argue that CBT helps people with this illness (as it does in every chronic disease model ever tested) but does not cure the illness.

"It is easy to pick apart any study after the fact, but put this in the context of the time (2001 when it was written) and place (the US where we were absolutely obliged to use the CDC case definition if we hoped for funding at the time.)

"I am very proud to work with a multidisciplinary team that includes experts in autonomic function, immunology, cell biology, psychoneuroimmunology, genomics, systems biology, nutrition, measurement illness state… the list goes on. Its a very exciting time and the team is very much jazzed and wants to see the work we do in all of these areas lead to both a clear understanding of the illness at the cellular level and then to effective treatments. You are going to see many papers this year from the different members of the team and taken separately you may fail to see the big picture. Don’t get sucked into too narrow a view. Wait and see… some amazing science is underway."


After which nklimas added a second comment:

"I had said that I had pressed Dr Antoni to get this study published in large part to rebut Dr White’s claim to cure 25% of ME/CFS patients with CBT. Dr Antoni’s study shows that while CBT helps people with the illness as it does in every chronic disease model ever tested, it does not cure the illness.

"kind of key first line – right?"

That surely settles it, doesn't it?! Well...

5. Practical Lesson 1 on Bullshit - B. Logic for dummies

So let me draw on my own toolkit - see: On natural philosophy, philosophy of science, and psychiatry - that seems far too difficult to master, even partially, for medical doctors and psychologists, and indeed it happens to be true that I have never met anyone of any of these groups who had done so, which, as you may understand, worked miracles for my faith in these persons.

And let me not just draw on it, but apply it. I quote in blue, as before, and with indentation, and comment in black, without said article:

Gosh! "MD, Medical Director, clinician, researcher, leader, advocate and educator": She sure had no time 'to do ME'! All served to this naive reader in the panting, grandstanding way ad miracle workers use to write their copy, including such fallacies, very common in ads, as "forgetting" to add "to do even more" than what the more would be, and also by brapadding terms like "even", that should deeply impress the braindead.

She decided that the only way to see more patients and train more doctors was to open a nationwide network of clinics that would take seriously both the treatment of patients with Chronic Fatigue Syndrome (CFS) but also begin to fill the void of trained clinicians by training them herself.

Wow, wow, and wow!

Why she "decided" that, she does not deign to say, to such enormously stupid folks like me; why her decision would be "the only way" she also fails to reveal; and why it would be a good idea for someone with no less than seven functions and four professorates to also "fill the void of trained clinicians by training them herself" also is hidden in the dark.

Now I am obviously willing to give all manner of deserved credit to professors Klimas - but here I seem to read ad copy, and not the limpid, honest, clear, calm prose of a capable rational scientist. But yes, people do medicine for the money, is true...

These clinics would follow the methodologies created by Dr. Klimas in her current clinic, using solid, evidenced- based medicine to treat this unique population.

Buddha, Jesus and Mohammed also started small, we must all remember, but this philosopher of science should very much like to know what are these "methodologies created by Dr. Klimas". She does not tell, nor does she tell how, inbetween professoring her four learned subjects, and working on her seven functions, she did "create" "methodologies".

Besides... as I outlined before, I - personally, handicapped by a very clear mind, to be sure - can't really believe that "medicine" that is presented as "solid, evidenced-based" is what it is claimed to be, for similar reasons as I have some doubs about a second hand cars salesman who enthusiastically praises his very own honesty: Should not be needed, if relevant and true; shows itself, rather than boasts itself, as the logically thinking minds know:

Manipulative propaganda terminology like "evidence based science" and "evidence based medicine" are pleonasms in real science and real medicine, and therefore are only, but then indeed ad nauseam, used by pseudoscientists.

My strongly evidence based finding about "evidence based science" is that - in modern times - all and only pseudoscientists insist that the pretentious bogosity they peddle is "evidence based science".

Indeed, I know of no work of real science - physics, chemistry, mathematics - that insists it is "evidence based science".

It is quite like honesty: If it is so, it goes without saying; if honesty is insisted upon, it likely is not honest.

And so with the pseudosciences - clinical psychology, psychiatry - that insist they are 'evidence based': The reader is consciously fed bullshit.

But back to professors Klimas very many excellencies, that are sung so beautifully on her very own site (and e.g. much more beautifully and copiously than I sing mine, on my site, but then indeed I don't make money by medicine or psychology):

A leader in the field of Immunology and an expert in Chronic Fatigue Syndrome (CFS), Dr. Klimas is the immediate past President of the International Association for Chronic Fatigue Syndrome.

That is the second time we hear of the empowherment as "leader". Now I am not saying all of this is bullshit, but pray... what is "A leader in the field of" some science? I grant kindly that it sounds very well, if one cannot think to clearly, but although I am 60 and have read a lot of science and scientific philosophy, I do not know of "leaders", in real science.

Examples, inspirers, thinkers, great scientists, geniuses - yes... I have read and heard of those. But "leaders"?! Is science perhaps a sport at the University of Miami, Miller School of Medicine? Or has it turned to politics? Or business? Professors Klimas do not tell, but do tell - with great pride, I suppose, as psychologist:

Did I mention four professorships? That's more than Newton, Euler and Einstein combined, readers! Or is it perhaps one professorship in something grandiloquently dressed up by four terms? Or is this perhaps an example of "if you have it, flaunt it"? Or - perhaps - it is a very provincial School of Medicine? Anyway: She also is "principal investigator" albeit of not more than "one of (..) three " etc. Furthermore...

At the University of Miami, Dr. Klimas leads a multidisciplinary research team representing the fields of immunology, autonomic medicine, neuroendocrinology, behavioral psychology, rheumatology, nutrition, and exercise physiology."

I am glad the copywriter got it right that a professor at the University of Miami is - by God's miraculous good grace - a professor at "the University of Miami", and am once more enormously impressed by the fourfold professors with at least seven fields of professional excellency that she also is expert (no doubt, dear reader!) in again seven fields of expertise (no doubt, dear reader!).

Anyway... now we all know who professors Klimas are, and know of the very many excellencies, expertships, studies and what not she had done or does, let me once more notice what all that excellence helped produce, at "the University of Miami":

See?! You can trust professors Klimas!! No? Let's consider an equivalent of sheer genius, unfortunately hiding its excellencies and gender behind a humble anonymity, as if he/she/it fears being satirised by name. This brave person let the world know that

This clearly, evidently, patently, and certainly established with divine certainty that "Dr Klimas" cannot possibly be mistaken about anything, and never lied or bullshitted. Cross my heart! Therefore, you see, "CBT helps people with the illness ME": Believe dr. Antoni and dr. Klimas! Twentyfive years of honesty and 'evidence-based medicine' guarantee! Moreover, ye skeptics and unfaithful...

She is compassionate and truly understands the disease

I do not say no, though it surely helps if one has "the disease" for the 33rd year, as a psychologist and analytic philosopher also, that indeed may not qualify me as - say - 'immunologist' (one of the very many subjects professors Klimas mastered and teaches, professorially), but that certainly does qualify me to pick apart bullshit:

and will fight the patients doctors to death.

This is most heartening to know: "compassionate" and "will fight the patients doctors to death". Literally, figuratively or a la mode de bullshit, my dear brave superintelligent Anon?

She has volunteered on a number of committees like CFSAC and has a number of relationships with key people in the community to help ME/CFS patients. For that I say thank you.

Gosh! And: Golly! All from the great goodness of her heart, and without any ulterior motive to become "a leader", or improve her bank balance? It is logically possible, I grant, fairminded as I am.

We arrive at the morals (in cauda venenum est: Professors Klimas can translate, no doubt):

Instead of tarnishing the reputation of one of our rare, awesome doctors, one would be better off trying to be an advocate for our cause.

Am I "tarnishing the reputation", if I say that the prose that "one of our rare, awesome doctors" uses is bullshit up, bullshit down, bullshit left, and bullshit right, indeed also embellished by rare morsels of a possible factual nature?

Doesn't she do it herself, rather? I really know little about professors Klimas, but I have read parts of her site and seen some of her videos, as reported in my Warning for the cfsKC site - that advertises professors Klimas as if she is Manna from High Heaven, against prices I cannot afford for even the simplest things on offer, and that very explicitly claims it is

Sorry mates: There you really err. I don't write this manner of prose, for one thing.

Also, I admit I am getting tired of this, so I will - for the moment - be fairly brief and partial with professors Klimas very own EM-prose:

The list may go on, but if one is an "expert" in "psychoneuroimmunology" or "measurement illness state" one is an expert bullshitter. I am sorry, but I call it as I see it, though I grant postmodern universities have "educated" - so they say - up to 50% of the total population, which led to the need of inventing all manner of "academic" "expertise", "subjects", "studies", and "academic" bullshit, in brief, to provide the majority of these "academically educated"  braindead with their own field of "academic expertise": MBA! Communication studies! Media studies! Empirical theology! European Studies! Etcetera ad nauseam! 

Its a very exciting time and the team is very much jazzed and wants to see the work we do in all of these areas lead to both a clear understanding of the illness at the cellular level and then to effective treatments." 

This patient is so thankful that in this very exciting time the team is very much jazzed up and even - gasp, Gasp, GASP! - desiring to lead to understanding and treatment. Wow! That must have cost many hours of meetings to arrive at as A Mission Statement. We stupid patients must be most thankful to that kind professor Klimas that a mind like hers is deigning at all to fatigue itself for patients.

But there is more from the "leader"

I doubt I am "going to see" these papers, simply because I don't believe in them, having considered the first. I may be mistaken, but will explain in a moment why I think not. This surely will not convince any member of "the different members of the team" but then I am assured, so kindly, so smartly, so politely, that I "may fail to see the big picture". So... if I disagree, it must be my fault, imperceptive as I must be. Also, I feel I am taken very seriously, at 60, by having a nanny kneeling beside me who assures me "Don’t get sucked into too narrow a view." Thank you, Miss! I must have dysfunctional beliefs, that I can't see the wood for the trees!

And then, after the masterful work of dr. Antoni, who established, with 'evidence based medicine', no less, that

CBT helps people with the illness ME

Trust dr. Antoni! Trust professors Klimas! All four of them!

Finally, to consider what it is all about. As one can't get the full paper, as an underbrained possible patient of professors Klimas, except by paying a lot of money, I must limit myself to what was quoted above, which I must reconstrue, because professors Klimas could not:

Really now. Why not simply answer this fraud - politely, of course: I do know academic habits and (im)postures - that it is immoral and quite possibly legally persecutable and not befitting for any practician of medicine to offer persons with a neurological disease "therapies" meant for the psychiatrically afflicted? Just as it is immoral and quite possibly legally persecutable to tell millions of ill people, and more millions of their friends, family-members that these persons with a neurological disease have dysfunctional beliefs that they are ill?

Professors Klimas, psycho doctor Antoni, and the team could not think of that answer, it seems.

Indeed, what they do claim is this:

I am sorry but I very much doubt it does - but then I have to either eat this week or starve and pay for the great fourfold professors Klimas paper, establishing that professor Peter Denton White, who is a bullshitter, a fraud and a liar, has been right all the time that

Professors Klimas and her team of pure evidence based medical geniuses proved it! So it must be true - or one gets Anon on top of one, complaining about one's morals, one's brain, one's knowledge, one's education, and one's personal integrity and sincerity, all without mentioning her/his/its name, titles, education, or any site from which one can infer that Anon must be a genius like Klinas and Antoni, if he/she/it is not their employee, of course.

So... what do I think really happened?

One doesn't know, for certain, in the midst of all this baloney and bullshit, but it seems to me that professors Klimas and professor White have nicely brought it about that the media may report that they both have found that

CBT helps people with the illness ME

(to me a statement on par with "psychoanalysis cures homosexuality", "psychiatry dissolves stomach ulcers", "Freud was an honest man", "witchcraft is science", and "pilgrimage to Lourdes cures all disease, just like LP"), while professor White has yet another argument to grab yet another 10 million pounds from the English taxpayers, this time to refute or critique - is the right name, in this milieu - professors Klimas's and dr. Antoni's paper.

And so everyone is a winner: Professors Klimas, with yet another notch in wherever she notches her "scientific papers" up; professor White, being taken serious instead of derided, with yet another prop for his manner of fraudulent bullshit; the members of the team, all with an academic credit-point to their names; the lot of them, for being very well paid and subsidized; the US and English state bureaucracies, with yet another reason to force people into CBT instead of investigating what really ails them, which is much more expensive if also the moral and honest thing to do; the health insurance companies, for having to pay far less in real medicines and real research of really ill people; and the parliamentarians and journalists, for now professors White and Klimas, and that amazing psychological genius dr. Antoni, have collectively proved that CBT helps people, also if they have a real neurological disease, or indeed anything else, again just as the American Psychiatric Association also teaches: Psychotherapy helps with everything! Pay the psychos to help the states keep the billions to pay to their friends the bankers!

Except for... the patients - but then, if CBT helps them, in the end they are really crazy anyway, as professors Klimas and White may prove eventually:

Wait and see… some amazing science is underway.

Incidentally, another advertisement slogan: Real scientists are quite capable of telling what amazing science is underway, that is, if they are not making it up.

Well... as the patients are poor, cognitively challenged, and clearly only worth bullshitting to, fuck them! Who cares?

(*) I am not of that kind, nor is my direct family, all intellectually gifted as well: Etiamsi omnes, ego non. That is also the reason why I wrote On a fundamental problem in ethics and morals.

(**) One reason for this impopular position that I am very intelligent (no desert: genetics); another that one also doesn't insist to a grandmaster of chess that he or she is just as good at chess as oneself is, if one is no grandmaster; a third is that intelligence or its absence really makes a difference; and the reason to warn you with this note about my deviances from the PC norm in giftedness and utterances is that you can often recognize the pomo-ness of an anonymous contribution by the insistence that all theories like all men are equal, equivalent, and as good as any other.

In politics, in religion, in the media, in "science", in advertisement ... you name anything of any social importance in the West, and if you look at it more closely you'll find in these blessed postmodern times everything of social importance - if some bullshitter can make money from it - is surrounded by hordes of professional bullshitters, and ensconced on the internet in masses of the bullshitting drivel these produced.

Just look at almost any university advertising itself - bullshit; any Dutch municipality's propaganda - bullshit; any "scientific" paper of Wessely, White, Reeves, or Bleijenberg - bullshit; any site of any commercial firm - bullshit; any daily paper, even those nominally addressed to "intellectuals" - bullshit, and indeed intellectuals is a species that has effectively been exterminated by postmodernism, which then resurrected it, ironically of course, as its parody and travesty, namely in the form of bullshitting quasi-"intellectuals" with Ph.D.'s and professorates in empirical theology (?!) with application to human rights (?!?!) and very many more pseudosciences like this. (And no: I ain't bullshitting you: Holland is blessed with these. And it pays VERY well, for doing NOTHING at all, except producing vast quantities of bullshit filled with cant embellished with crap. Mundus vult decipi.)


P.S. Corrections must wait till later.

But - speaking for myself, having also the academic diplomas for it - I do insist that anybody who writes or speaks what I consider bullshit about a subject I consider serious, and especially anybody who writes or speaks what I consider bullshit about science or who pretends that his or her bullshit is science, is my enemy.

For in general, and in science especially: Whatever can be said, can be said clearly - and whoever talks or writes advertisement lingo, and presents it as if that is "science", could be "science", should be "science", or merits serious, measured, polite, reasonable rebuttal, is bullshitting me, and can expect satire and derision in reply. To speak with Jon Stewart:

Go F...bleep...ck yourself!"

I ain't stupid enough to be taken in by your kind of baloney! And I don't believe any scientific pretension of anyone posing as a scientist who produces reams and reams of advertising bullshit - and I counsel the same to anyone else, for science has the great merit that it can and should be done rationally and with real respect for the facts, and be written and taught in an objective, honest, rational manner that is totally incompatible with any manner of bullshit.

That is also why it makes me so angry: It is in science and only in science that humans had succeeded to create a tradition of rational, objective discourse, where only one's intelligence, knowledge and the verified facts were relevant to one's claims, and where bullshit was banned, in principle, and until postmodernism tried to poison all the sources of real knowledge, mostly succesfully, to the great benefit of mankind - for the technologies on which human societies depend, are for the most part discovered by science.

And while one often simply lacks the knowledge to judge the science - surely dr. Klimas knows more of medicine than I do, for example - it does not require extra-ordinary intelligence to recognize when and how someone is bullshitting you, in the name of science.
